The pill is approved for adults. Some broader Wegovy indications belong specifically to the injectable formulation, including weight reduction in eligible adolescents and treatment for certain adults with noncirrhotic MASH. Patients should not assume that every Wegovy indication applies to both forms.
Dosing and Daily-Routine Differences
How Foundayo Is Taken
Foundayo is swallowed whole once daily and can be taken with or without food. The FDA-labeled schedule begins at 0.8 mg daily. After at least 30 days, the dose increases to 2.5 mg, followed by 5.5 mg after at least another 30 days. A clinician may then consider 9 mg, 14.5 mg, or 17.2 mg at intervals of at least 30 days, depending on response and tolerability.
The maximum approved dose is 17.2 mg once daily. Moving up faster is not a safe shortcut. Gradual escalation is intended to reduce gastrointestinal side effects.
How the Wegovy Pill Is Taken

The peer-reviewed ATTAIN-1 study of orforglipron was a phase 3, multinational, randomized, double-blind trial. It enrolled 3,127 adults with obesity who did not have diabetes. Participants received one of three doses of an investigational orforglipron formulation or placebo for 72 weeks, along with healthy eating and physical activity support.
In the intention-to-treat analysis, which included all randomized participants and accounted for treatment interruptions, average weight change at 72 weeks was:

- 8.4% with the middle studied dose
- 11.2% with the highest studied dose
- 2.1% with placebo
At the highest studied dose, 54.6% of participants lost at least 10% of their starting weight, 36% lost at least 15%, and 18.4% lost at least 20%. Gastrointestinal effects were the most common adverse events. Adverse events led 5.3% to 10.3% of participants in the orforglipron groups to discontinue treatment, compared with 2.7% in the placebo group.
There are two details patients should understand. First, the trial excluded people with diabetes, so its results do not describe every future patient. Second, the published trial used an investigational formulation and dose strengths. The FDA label presents corresponding results using the commercial Foundayo strengths, with an 11.1% average reduction at the highest approved dose in adults without diabetes.
Wegovy Pill Results From OASIS 4
The peer-reviewed OASIS 4 study of oral semaglutide 25 mg was a randomized, double-blind, placebo-controlled trial conducted at 22 sites in four countries. It enrolled 307 adults without diabetes who had obesity, or overweight plus at least one obesity-related condition. Participants received oral semaglutide or placebo alongside lifestyle intervention.
At week 64, the estimated average weight change was:
- 13.6% with oral semaglutide 25 mg
- 2.2% with placebo
The current FDA label also reports that 76.3% of participants assigned to the Wegovy tablet lost at least 5% of their starting weight, 59.8% lost at least 10%, 47% lost at least 15%, and 27.9% lost at least 20%. Gastrointestinal adverse events occurred in 74% of the oral semaglutide group and 42.2% of the placebo group.
Does This Mean the Wegovy Pill Works Better?
Not necessarily. The 13.6% average in OASIS 4 is numerically higher than the 11.2% average in ATTAIN-1, but this is an indirect comparison. OASIS 4 was much smaller, assessed the primary result at 64 weeks, and studied a different population and medication. ATTAIN-1 enrolled more than 3,000 participants and followed treatment for 72 weeks.
The trials were designed to compare each drug with placebo, not with the other medication. No randomized head-to-head result in these pivotal publications establishes that Wegovy is more effective than Foundayo, or vice versa. Individual response may differ substantially from the average.
Side Effects: Where the Two Pills Are Similar
Both medications commonly cause digestive symptoms, particularly during dose escalation. These may include:

- Vomiting
- Constipation

Do Not Use Either Medication If You Have Certain Thyroid-Cancer Risks
Both labels include a boxed warning regarding thyroid C-cell tumors. Neither medication should be used by someone with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia syndrome type 2. It is not known whether these medications cause thyroid C-cell tumors in humans.

Foundayo can affect oral hormonal contraceptive absorption. The label advises patients using oral hormonal contraception to switch to a nonoral method or add a barrier method for 30 days after starting Foundayo and for 30 days after every dose increase.

Wegovy’s strict morning routine creates a practical interaction with other oral medications. It also delays gastric emptying and may affect drugs that require close monitoring. For example, the label reports increased levothyroxine exposure in a tablet interaction study, so thyroid medication timing and laboratory monitoring may need review.
Pregnancy and Breastfeeding
Weight-loss treatment offers no benefit during pregnancy and may cause fetal harm. Both drugs should be discontinued when pregnancy is recognized when they are being used for weight reduction. The Wegovy label advises stopping at least two months before a planned pregnancy because semaglutide remains in the body for a long time.
